With the awareness of this responsibility, we process your personal data as a Data Controller within the framework specified below in accordance with the Personal Data Protection Law No. 6698 (Hereinafter referred to as the Law) and the relevant legislation.

Within the scope of the Law, data processing refers to any operation performed upon personal data such as obtaining, recording, storing, updating, classifying, sharing with or transferring to third parties permitted by the legislation, completely or partially automatically, or non-automatically provided that it is a part of any data recording system.

COLLECTION, PROCESSING AND PURPOSES OF PROCESSING OF PERSONAL DATA

In order to provide you with high standard services, we process your personal data within the scope of the purposes specified below, depending on the nature of the service provided:

  • To provide the treatments and services offered at ZEYNEP YILDIRIM Oral and Dental Health Clinic within the determined legal framework,

  • Your identity data such as your name, surname, TR identity number, your passport number or temporary TR identity number if you are not a Turkish citizen, your place and date of birth, marital status, gender information, and a photocopy of your TR Identity Card, Institution ID, or Driver's License you submitted,
  • Your contact data such as your address, phone number, e-mail address,
  • Your financial data such as your bank account number, IBAN number,
  • Your health-related data obtained during the execution of medical diagnosis, treatment and care services, such as your laboratory and imaging results, test results, examination data, prescription information you provided to be followed up in your file,
  • Health report and certificates and documents requested within the scope of occupational safety,
  • Your responses and comments you share in order to evaluate our services,
  • Your data regarding private health insurance and SSI data for the purpose of financing and planning health services,
  • Your closed-circuit camera system image recording taken during your visit to our clinic,
  • Navigation information, IP address, browser information obtained during the use of our website, and medical documents, surveys, photos, form information, and location data you provided with your consent.

Your personal data and special category personal data listed above may be processed within the personal data processing conditions specified in Articles 5 and 6 of the KVK Law and for the purposes listed below:

  • Sharing the requested information with the Ministry of Health and other public institutions within the scope of the relevant legislation,
  • Protection of public health, preventive medicine, execution of medical diagnosis, treatment, and care services,
  • Providing training to our employees by our KVKK consultants,
  • Answering all your questions and complaints regarding our health services by clinic management and personnel,
  • Taking all necessary technical and administrative measures within the scope of data security of systems and applications as ZEYNEP YILDIRIM Oral and Dental Health Clinic,
  • Fulfilling legal and regulatory requirements,
  • Confirming your identity by our patient services department,
  • Informing you about your appointment through our personnel,
  • Sharing the requested information with private insurance companies within the scope of financing your health services, covering your examination, diagnosis, and treatment expenses by patient services and financial affairs departments, and eligibility inquiry,
  • Planning and managing the internal functioning of the institution by the management of ZEYNEP YILDIRIM Oral and Dental Health Clinic,
  • Providing training to our employees by Human Resources,
  • Monitoring and preventing abuse and unauthorized transactions by Audit and Information Systems departments,
  • Performing risk management and quality improvement activities by Information Systems departments,
  • Invoicing for our services by Patient services and Financial Affairs departments,
  • Confirming your relationship with institutions contracted with our company by Financial Affairs departments,
  • Designing and delivering special content, tangible and intangible benefits on Media and Communication, Web and mobile channels,
  • Measuring, increasing, and researching customer satisfaction by the relevant departments of our company,
  • Carrying out education and training activities by educational institutions that the institution cooperates with.

Your "Personal and Special Category Data" mentioned above will be kept with great care and in compliance with the provisions of the legislation in physical and electronic archives within ZEYNEP YILDIRIM Oral and Dental Health Clinic and external service providers.

TRANSFER OF PERSONAL DATA

Your personal data will be shared with our business partners, suppliers, ZEYNEP YILDIRIM Oral and Dental Health Clinic officials, legally authorized public institutions and private persons, and the following parties within the framework of the Basic Law on Health Services No. 3359, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Institutions, the Personal Data Protection Law No. 6698, other legislative provisions and for the purposes explained above

  • Your legal representatives whom you have authorized,
  • Laboratories, medical centers, medical device and healthcare institutions at home and abroad with which we cooperate for medical diagnosis and treatment,
  • Third parties from whom we receive consultancy, including lawyers, tax consultants, and auditors with whom we work,
  • Regulatory and supervisory institutions and official authorities,
  • With your employer,
  • With our suppliers, support service providers, archive service providers, and business partners whose services we use or with whom we cooperate

It can be shared within the framework of the personal data processing conditions and purposes specified in Articles 8 and 9 of the KVK Law.

METHOD AND LEGAL REASON FOR COLLECTING PERSONAL DATA

Your personal data is collected and processed in all kinds of verbal, written, visual or electronic media in order to carry out all kinds of work included in the activity subject of ZEYNEP YILDIRIM Oral and Dental Health Clinic within the legal framework and in this context, for ZEYNEP YILDIRIM Oral and Dental Health Clinic to fully and properly perform its employment contract and legal obligations. In addition, as stated in Article 6/3 of the Law, personal data related to health and sexual life can only be processed by persons subject to confidentiality obligations or authorized institutions and organizations without seeking the explicit consent of the person concerned, for the purposes of protecting public health, preventive medicine, medical diagnosis, treatment and care services, and planning and managing healthcare services and financing.

YOUR RIGHTS REGARDING THE PROTECTION OF PERSONAL DATA

In accordance with the Law and relevant legislation;

  • Learning whether personal data is processed,
  • Requesting information if personal data has been processed,
  • Accessing and requesting personal health data,
  • Learning the purpose of processing personal data and whether they are used appropriately for their purpose,
  • Knowing the third parties to whom personal data are transferred domestically or abroad,
  • Requesting correction of personal data in case of incomplete or incorrect processing,
  • Requesting the deletion or destruction of personal data,
  • Requesting that the operations regarding the correction of incomplete or incorrect processing and/or the deletion or destruction of personal data be notified to the third parties to whom the personal data has been transferred,
  • Objecting to the occurrence of a result against the person himself/herself by analyzing the processed data exclusively through automated systems,
  • You have the right to demand the compensation of the damage in case you suffer damage due to the unlawful processing of personal data.

DATA SECURITY AND RIGHT OF APPLICATION

Your personal data is meticulously protected within technical and administrative possibilities, and necessary security measures are provided at a level appropriate to possible risks, taking into account technological possibilities. Pursuant to paragraph 1 of Article 13 of the KVK Law, you can forward your request regarding the exercise of your rights specified above to us as ZEYNEP YILDIRIM Oral and Dental Health Clinic in writing or by other methods determined by the Personal Data Protection Board. To exercise your rights specified above, your request containing the necessary information establishing your identity and your explanations regarding your right that you request to exercise from the rights specified in Article 11 of the KVK Law;

Depending on the nature of your request, it will be concluded free of charge as soon as possible and within thirty (30) days at the latest. However, if the transaction requires an additional cost, the fee in the tariff determined by the Personal Data Protection Board will be charged by us.
